The Loose Crank Pulley

I've been reading a few threads about people who have had the problem with the loose pulley. Now I'm constantly checking on mine (2300 miles), they are pretty tight.

My questions to you guys are, what causes the problem and how to prevent it? How often is this problem occurs? I've read a thread that a guy only have less than 2000 miles.

How come Dodge didn't fix the problem?

I hear alot of SRT10 Ram owners having the same problem checked mine multiple times and no problem yet.
