common manufacturing issue?

In the process of rebuilding a 57k mile 2003 viper for a friend and found #4 piston installed backwards. (IE the piston/rod combo was for the opposite side of the motor.) I also discovered a large piece of casting flash not removed from the lifter bores. The shard of metal literally fell off when touched. The whole reason this engine is being rebuilt is the common 3/4 rod journal issue, but needless to say I am quite surprised at the lack of quality control.

Just wondering if others have had similar findings...

I have yet to see something like that regarding the rod. The flashing happens, but rarely. Very rarely. 9/10 it would be a piece that will never fall off by itself, or is too large to get sucked into the sump regardless.

The Piston & Rod... it would surprise me one bit if someone worked on the engine later and pulled that fast one.

It had me questioning my technique of dis-assembly when I noticed it was oriented wrong. I then counted all of them once they were out and had 6 and 4 of each orientation. The block to the best of my experience has never been taken apart before, therefore it happened at the factory.

It would not surprise me in the least. I have worked for a major space electronics company. It is really surprising how easy it is for human beings to make a mistake and others to miss catching it, even when they know how much the stuff costs and how critical the parts are to the final product.

I wouldn't think that even a high end auto manufacturing line would be immune.
